Move Better & Do More

Physical activity is a key component to wellbeing journey. Leading a physically active
lifestyle positively impacts your ability to cope with daily stressors, move through your
environment, and complete activities of daily living.

Level 2 MOVE & DO Programs

These simple to complete programs are intended to engage participants with important but brief information about many different health topics. Full-Time participants will be incentivized for selecting and completing any three Level 2 programs ($100 each).

One Small Change

Long-term healthy living needs simple beginnings. One Small Change is a health behavior change program that encourages participants to identify one health behavior that they would like to adopt, modify, or eliminate and live that change for at least 21 days. Support for identifying a change, as well as living the identified change, comes from attending and participating in small group discussions.

Prioritization is given to those who have not yet participated.

Recipe Book and Mindful Meals: This program is designed to further the connection between meal planning, shopping, cooking, and eating.  Participants will prepare, consume, and evaluate 10 recipes from either of the custom-built SLCC's recipe books.  Within the recipe book, participants will have the opportunity to learn how to incorporate mindful eating practices into their daily lives.

This program is asynchronous and begins when you want it to begin and ends when you have completed the Recipe Rating Tool. Please download and complete the Recipe Rating Tool. To receive incentive request instructions, send your Recipe Rating Tool file to Christina McWhinnie.

Level 4 MOVE & DO Programs

This level is independent of the previous three levels. These programs are intended to help prevent the onset of or better management of chronic conditions. Participants will spend the entire fiscal year within this level. The full annual incentive amount will be distributed throughout the fiscal year. Please complete the program from this list that is most applicable for you:

Chronic Disease Self-Management Program

These Level 4 Employee Wellness programs are designed to help individuals living with chronic conditions to improve their quality of life by improving the quality of their self-care routines.  Full-Time employees interested in better managing their chronic condition(s) are welcome to register for one of the LivingWell with Chronic Conditions offered by our community partners.

Chronic Disease Self-Management Program (CDSMP): The CDSMP has shown to be effective in improving: exercise, cognitive symptom management, communication with physicians, self-reported general health, health distress, fatigue, disability, and social/role activities limitations among individuals living with chronic condition(s). The CDSMP is a 6-week (2 hours per session) program with a 12-week aftercare (1 hour per session) extension. Participation in this program requires a medically diagnosed chronic condition (e.g. arthritis, asthma, diabetes, lung disease, heart disease, stroke).

Diabetes Self-Management Program (DSMP): For diabetics, the DSMP aims to increase participant’s confidence in their ability to manage their health and maintain active fulfilling lives through the implementation of highly participative and supportive small group discussions. The DSMP consists of three components: CDSMP, DSMP, and a 6-week aftercare program. Participation in this program requires medically diagnosed diabetic status.

Aftercare Program: The Aftercare Program is designed to extend the support received through the CDSMP and DSMP programs.  Depending on your unique situation a 6-12 week aftercare program will be required to receive the full level 4 incentive.  Contact an Employee Wellness Benefit team member to register for an aftercare program.

It’s important to get a wellness checkup every year because many health conditions don’t cause symptoms until they’re serious. Most serious health conditions and diseases can be prevented or are easier to treat if you catch them early. Here is a guide to help you and your family know what screenings you need.
